I've always had the same problem as well, because the NHS also does good investigations for many causes, I was very annoyed when on one day they found out we don't record it anywhere as they had made complaints over 10s the previous ones about staff talking behind or under bodies in mortuaries etc

They never did one of their own investigation

What they did was simply publish what their own system thought to a few papers as though it's in-world

Then someone made some fake accusations and wrote a stupid one down on paper "Mum" (I use quotation marks there a

to avoid accusations) as being a paedophile

And this caused some big issues when these accusations took off within hours of the paper coming down - so these newspapers were forced to back down (that happened several times a day over their front editions)

And of course some reporters took their word and got all this stuff out and in their blogs and stories etc.... they also reported on some 'ex' people that weren't 'their case studies', this all did to some great results to
